Care Management

A Care Manager is a health and human services specialist who, through a collaborative process, helps families caring for older relatives manage physical, social and medical health conditions more effectively. Our staff of experienced professionals act as advocates--identifying and prioritizing problems, offering solutions, and providing continuous client evaluations. By improving the coordination of care, we are able to assist in providing cost effective, non-duplicative services. Our goal is to help the client achieve an optimal level of wellness, and the preservation of independence, dignity and self esteem.

Care Management Services include but are not limited to the following:

• Scheduled visits by our professional Care Manager
• Coordination of Home Health or Hospice
• MD/Dental appointment arrangements
• Coordination of Physical/Occupational Therapy
• Medication management with a local pharmacy (including delivery)
• Aide in providing assistive devices
  (such as wheelchairs, shower chairs, grab bar installation and all safety equipment)
• Transportation services
• Coordination of:
• Gardening and pool services
• Handyman services
• Housekeeping services
• Pet assistance needs
• Notary services
• Referrals to:
• Elder and estate lawyers
• Local real estate professionals
• Needs assessment and recommendation for placement in local assisted living facilities or skilled nursing facilities
• Insurance contacts for equipment coverage
