Care Management

Professional Geriatric Care Management is performed by a Registered Nurse or a Certified Senior Advisor experienced in gerontology, rehabilitation, and hospice. This program assists older adults and their families in attaining their maximum functional abilities. We act as advocates for you and your family—identifying problems and offering solutions. We provide you with peace of mind that comes with the coordination of a trusted network of preferred providers, support groups and professional services. Our goal is the preservation of independence, dignity and self esteem through continuous evaluation of your total quality of life.

Services included, but not limited to the following:

• Coordination of home health or hospice
• Scheduled Nurse Visits
• Nurse and Physician one on one communication
• MD/dental appointment arrangements
• Coordination of Physical/Occupational Therapy
• Medication management with local pharmacy including delivery
• Assistance providing and installing safety equipment such as grab bars, shower chairs
• Transportation services
• Coordination of handyman services
• Coordination of housekeeping services
• Referrals to local real estate professionals
• Needs assessment and recommendations for placement in local assisted living and/or skilled nursing facilities
• Insurance coverage for equipment
• Notary services
• Coordination for pet assistance needs
