My Personal Care Plan


Personal Care Plan:

We offer a flexible consultation schedule for your convenience. We will come to your home for a free in-home assessment and safety inspection. Your needs and those of your family are unique. Our home care coordinators will assist you in choosing which services are most appropriate for you at the time. From there, a confidential and personal care plan will be formulated and designed to improve your quality of life while meeting your unique needs.

We are available 24 hours a day, 7 days a week to assist you. We have a three hour shift minimum and no weekly minimum requirement. You decide what days and hours work best for you.

Our Employees:

We carefully select and match our employees with your needs to insure the “best fit” for your care. You can have confidence in knowing that our employees are bonded and insured, covered by our company’s worker’s compensation insurance, and that payroll taxes are reported and paid. You should also be assured that all of our employees are screened, personally interviewed and subjected to a rigorous background check. We screen our employees for any state or federal criminal activity and for any DMV code violations and repeat both screenings annually on anniversary date of hire.

We verify prior employment histories, and any references and former residences are reviewed by us as well. In addition to yearly TB testing, we maintain very strict anti-alcohol and drug policies.

Continuing education is provided to all caregivers through mandatory orientation training and in-services, monthly educational classes, CEU classes, as well as specialized Level 3 training in Hospice, Alzheimer's/Dementia and Parkinson's.

In The Beginning:

Once services start, you will receive a follow-up call from our office staff or a Care Manager to make sure everything is going according to your confidential care plan and all questions and concerns, if any, have been answered and allayed.

During Your Care Period:

We make every effort to stay proactive in satisfactorily meeting your needs through occasional follow-up phone calls to you and by keeping in regular contact with our caregivers. We encourage you to call our office at any time should any questions or concerns arise. You will also receive periodic visits from one of our Care Managers to make sure the care you have received to date is as per the care plan. In some cases, a reassessment of care may have to be made resulting in a change to your care plan.

Billing Throughout Care:

You will be billed twice per month for our services. You will not be responsible for paying the caregiver, we are responsible for this. We accept most credit cards and will help you with any forms you need to fill out in order to file a claim for long term insurance coverage.
